Easy access to your personal health information.
The Foothill Cardiology team is proud to introduce our on-line health portal for our patients. Through this portal, patients can access and manage their health demographics, insurance information, exam details and communicate with your provider electronically.
Our patient portal is a protected site where only you or individuals you designate can have access to your information. It’s easy, user friendly, and designed to help you stay informed, healthy and safe.
PATIENT PORTAL
Follow these easy steps to access your Health Portal. There you can create your profile, manage your settings and customize your password.
1. During your visit, provide your personal email address to any of our staff members.
2. You will receive a token which is used to access your on line portal. You will also receive an email with the link to the portal for easy access.
3. Click the Patient Portal logo at the top of the page to access the login screen.
4. Use your email address as your username and choose your password.
If you have an urgent problem that occurs after hours or on the weekend, phone the office where you are seen, and you will reach our exchange. You will either be asked to stay on the line, or they will take your number and notify the on-call physician. Doctors alternate evening and weekend emergency calls, enabling us to always have a physician available.
In a true emergency, it is best to dial "911" (paramedics) and be taken to the emergency room of the nearest hospital where the physician on duty will begin treatment and phone our on-call physician.
EMERGENCY SERVICES
Our business office is happy to discuss with you any aspects of your bill or payment schedule. You can call and speak with one of the billing personnel or make an appointment to speak with them in person.
FEES
Your medical record is strictly confidential. We will not give out information regarding your condition to your employer, relatives or friends without your written permission. The only exception to this is when disclosure is required by law.
CONFIDENTIALITY
We value our relationship with you and believe it is based on mutual trust and understanding. We make a special effort to explain everything to you regarding your condition. However, if there is something you do not fully understand we encourage you to ask questions.
